I used an Albums Made Easy pocket sleeve from WRMK for this. I took these pictures when my son was 5, and had him write answers to some questions in his own handwriting. I could never find a way to incorporate them onto a page that I liked, until I started using these sleeves between my 12x12 layouts. They are so much fun to use! I made individual cards with some machine stitching to highlight our "Q & A". Sorry the pic isn't great quality, these pocket sleeves are a little too big for me to scan.
